PSA Dynamics And Nomograms In The Diagnosis And Treatment Planning For Prostate Cancer

UroToday.com - The introduction of PSA as a marker for prostate cancer has dramatically altered how prostate cancer is diagnosed and treated. Along with the improvements in diagnosis and treatment options has come the realization that as men age prostate cancer is relatively common and that not all prostate cancer is fatal and necessarily needs to be actively treated.

Almost as soon as PSA was introduced into widespread use, efforts were underway to increase diagnostic sensitivity and specificity, to identify those populations of men who would benefit from screening and to distinguish intermediate and high risk disease from the low risk variety. PSA can be elevated for a number of reasons and distinguishing benign from malignant elevations remains one of the most important areas of ongoing research.

This presentation will review the current options in use for improving the diagnostic accuracy of PSA and will include PSA-density, free and bound PSA, PSA-velocity and PSA doubling time. A discussion of which of these PSA based-options is most appropriate for specific situations will also undertaken.

In the post diagnostic situation the role of PSA and its variants as a prognostic tool will also be reviewed as will diagnostic and prognostic nomograms.

Recent abstracts from the American Society of Clinical Oncology and the American Urological Association meetings will be reviewed and the attendee should have a clear understanding of the role of these various modalities following this presentation.

Finally, the current options for prostatic biopsy and the benefits and problems with the expanded regimens are discussed.
