Contact Lens Care & Infection Prevention

Over the past two years, several popular contact lens solutions have been linked to cases of serious eye infections. The most recent case involved Advanced Medical Optical’s Complete? MoisturePlus? solution, which was recalled in May of 2007, as it was linked to cases of Acanthamoeba keratitis, an inflammation caused by a parasite. With the link between infections and contact lens solutions established, eye care professionals are examining the existing regulations of the Food and Drug Administration (FDA).
The Complete MoisturePlus solution was not contaminated, but was found to be ineffective at preventing the infection. The Centers for Disease Control (CDC) found that contact lens wearers were 17 times more likely to contract Acanthamoeba keratitis if they used Complete MoisturePlus than other multi-purpose solutions.
Acanthamoeba keratitis
Acanthamoeba are microscopic one-celled organisms that occur naturally in water sources such as tap water, well water, and hot tubs. Acanthamoeba are parasites capable of thriving in the neutral pH environment of the eye. If the eye becomes infected with Acanthamoeba, Acanthamoeba keratitis results.
The infection is rare, but can be severe and painful, often causing corneal scarring and sometimes blindness. Acanthamoeba keratitis symptoms include redness and eye pain after lens removal, as well as tearing, light sensitivity, and blurred vision. Because the symptoms are similar to those of other infections, Acanthamoeba keratitis can be very difficult to diagnose and treat. Some rare cases may even require a corneal transplant as treatment.
To avoid contracting the infection, it is important that contact lens wearers:
·Follow the recommendations of their eye care practitioner regarding contact lens products and care.
·Never use tap water to rinse or clean lenses or cases. Use only sterile solutions.
·Do not wear contact lenses while swimming, showering or in a hot tub without airtight goggles.
·Soak lenses in fresh disinfecting solution every night, never top off solution in lens case, and do not use homemade solutions.
·Wash hands before handling lenses.
·Keep lens case clean and dry when not storing lenses.
·Clean lenses upon removal and rub lenses with multi-purpose solution. Not all solutions have the same ability to disinfect, so be sure to speak to an eye care practitioner before making any changes.
It is important that all contact lens wearers keep in mind that contacts are a medical device, and improper care of their lenses can result in painful infections or other impairments.
Wearers must be diligent about keeping their contact lenses and case clean, and replace cases at least once every three months. Be sure to only use sterile solutions recommended by an eye care professional.
Recommendations
In response to the recall of AMO’s Complete Moisture Plus solution, a panel of experts recommended that the FDA impose stricter guidelines for testing and labeling contact lens solutions.
The American Optometric Association suggested the FDA strengthen its testing of products before introducing them into the market.
“We feel that a more standardized testing process should be developed and used by the FDA, prior to approval, as well as to compare efficacy between products so that a practitioner can make better judgments when prescribing solutions,” Dr. Louise A. Sclafani, chair of the American Optometric Association (AOA) Contact Lens and Cornea Section, said in a presentation to the FDA.
Some AOA recommendations included testing under no-rub and no-rinse conditions and testing with actual lens and case materials.
A study by the University of Manchester in the United Kingdom found that most contact lens wearers do not wash their hands before inserting their lenses, and are less likely to do so when removing them. Another study found that risk for infection increases by four times when wearers do not properly clean their cases. The AOA also suggested that tests be performed with dirty hands and dirty cases.
The FDA regulates contact lenses through the Federal Food, Drug and Cosmetic Act. The active guidance document relevant to contact lens products was drafted in 1997. The AOA recommends updates to labeling to include a mandatory discard date. Labels should also include, in a prominent, large font, disclaimers such as ‘Wash hands before handling lenses or products,’ ‘Do not top off solution,’ and ‘Rub and rinse is necessary,’ the AOA argued.
The FDA has not yet made any changes or updates to their regulations regarding contact lenses and contact lens products.

Dancing with Horses, Delia’s Story

Through my work as an Equine Experiential Learning Facilitator and Life Coach, I have been blessed to watch the magnificent transformation in the lives of countless individuals. Working with horses in partnership to heal humans is the goal of the New 4H Club, Horses Helping Humans Heal?. I’d like to share one of these wonderful stories with the hope that you will identify with the journey described here and come one step closer to your own healing.

In our beautiful and serene barn setting in April of this year, a group of amazing individuals gathered together to learn. One story in particular seems worthy of sharing, as it, I believe, demonstrates the amazing abilities of our equine friends in helping us heal.

As Dan stepped through the portal of change (the gates of the arena), his declaration was to fill the empty space in his heart, and to feel whole again. His chosen partner was to be a 4-year old Morgan, Quarter horse cross named Delia*. Standing only 14.2 hands high, Dan was easily able to see over the top of Delia. The horse/human connection began immediately with Delia moving herself quickly into Dan’s space. She proceeded to rub herself all over him in what I perceived as an aggressive manner. This continued for many minutes as Dan steadied himself and allowed the rubbing to continue. Soon his jacket was covered with her buckskin hair and his jacket had, in fact, changed colour. The next thing that happened caused some in the audience to catch their breath, and my equine specialist to leap out of her chair and approach me with her concerns. In the process of Equine Co-facilitated Learning, timeouts for safety are strongly encouraged. Delia was literally chewing on Dan’s hand, and I could feel the strength of her jaws as she was grinding her teeth. All the while and this went on for some time, Dan just stood there smiling. Finally at one point, he removed his hand, and yes, there was a small break in the skin and extreme redness. Delia went back to rubbing herself on Dan’s jacket. I then asked Dan to step towards the outside of the arena structure, and to share his learning, as well as invite the interpretations of those watching.

While Dan got into his “story”, ignoring his red and somewhat swollen hand, he spoke of how much he thought Delia loved him and that she was just expressing her affection for him. At that point Delia moved over to me and pushed her head into my arm several times. I connected with her at the heart level and asked her what she wanted. She said the lesson was not over and that Dan needed to come back and work with her. The same dance was repeated, and this time after a few minutes, I interrupted. The role of the facilitator is a balancing act, insert yourself too soon and disrupt the process, wait too long and the learning may be lost.

I asked Dan if he was noticing how hard Delia was pushing herself into him and what that felt like? Did it remind him of anything in his life? As he stood with a smile on his face, I asked what congruency might be missing in him. What might your learning partner want to teach you? Where in your life is love painful? Where are your boundaries about what is acceptable to you? Who pushes you past the point of conscious awareness? Delia is teaching you something, what is it for you?

Dan then approached the edge of the arena and spoke of the pain in his heart, something that he had been unable to let go of. A relationship required closure, and he had been unable to walk through the pain. The smile on his face was replaced with the tears of sadness, and Delia walked over and placed her head on his shoulder, licking and chewing (a sign of releasing), and waited as he cried for the ending of something precious in his life that he was now willing to face. Through his partner and teacher, Delia, a 4 year old horse, he had been able to create a new congruency within himself as he began his “dance of healing.” Dan declared his action to go forward and have a conversation that would allow him to fill the space in his heart, and to feel whole again.

Dan called me several weeks later to say he finally “got” what Delia was trying to teach him. He had the difficult conversation with his previous partner, moved to a place of peace and acceptance, and finally was able to move forward in his life.

Mix It Up: Spinach, salmon, and more

I have a favorite salad I make at home. It’s healthy. It’s easy. It’s something you might want to add to your at-home menu.

It all starts with a bed of baby spinach leaves. Then I add a handful of red grapes, sprinkle on some shredded carrots, mix in some dried cranberries, and toss in a few shaved almonds. For a dose of good protein and a serving of essential omega-3 fats, I top with salmon, which is always a leftover from a previous dinner meal.

No dressing for this salad. The juice from the grapes and the moist fish do the trick all on their own.

Shortened telomeres cause us to age, and they also leave chromosomes vulnerable to damage, which can lead to diseases such as cancer. Exercise and you’ll keep these buggers fit for a young life.

Foods that make kids smart

I want smart kids, especially in light of my second-grader’s first science test score. Ouch. He didn’t do so well, which is a little hard to take since I’m his appointed tutor. Seems he could use some foods to build up that brain of his. I could use some too.

What a child eats has a direct effect on test performance. Gotta keep sweets and junk food to a minimum then and up the intake of the following foods.

  • High-quality carbohydrates, such as fruits, veggies, and whole grains
  • Lean protein, like turkey, chicken, eggs, and beans
  • Cereals, like oatmeal topped with fresh fruit or something with three or more grams of fiber and no more than six grams of sugar per serving
  • Snacks, featuring baby carrots, celery, pepper sticks, string cheese, nuts, low-fat popcorn, grapes, and berries
  • Skim or low-fat milk

There’s more to brain power than food. You’ve got to keep your kids active too — it improves circulation, which increases oxygen flow to the brain.

Obesity, Type 2 Diabetes Linked to Neurodegeneration

Researchers at Rhode Island Hospital say their study is the first one to show that obesity can cause neurodegeneration.

In a study on animal models, lead author Suzanne de la Monte, MD, MPH, of Rhode Island Hospital, utilized chronic high fat diets to cause a two-fold increase in mean body weight.

In these models, there was a marginally reduced mean brain weight and a significantly reduced mean brain weight/body weight ratio, providing evidence that obesity with T2DM is sufficient to cause mild global atrophy in the brain.

“In essence, the brain shrinks and several biochemical and molecular abnormalities found in patients with AD, including brain insulin resistance, develop with chronic obesity and T2DM. However, the extent of the abnormalities in no way matches AD,” De la Monte said.

Researchers found that the neuropathological abnormalities were mild and the associated brain insulin resistance could serve as a co-factor in the development and progression of AD.

Overall, the study showed that that the effects of obesity and T2DM can essentially aggravate or contribute to the severity or progression of AD, but cannot be the sole cause of the condition.

The findings suggest that strategies to reduce obesity and prevent or control T2DM could modify the clinical course of mild cognitive impairment and AD.

The study appeared in the Journal of Alzheimer’s Disease.

Memory Loss After Chemotherapy may Be Prevented by Antioxidants

During the study, rats were administered the commonly used chemotherapy drugs adriamycin and cyclophosphamide.

The researchers observed that the animals that were trained to prefer a light room to a dark room forgot their training while on drugs.

“When animals are treated with chemotherapy drugs, they lose memory. When we add NAC during treatment, they don’t lose memory,” said Gregory Konat, professor of neurobiology and anatomy.

Dr. Jame Abraham, director of the Comprehensive Breast Cancer Program at WVU’s Mary Babb Randolph Cancer Center, said that patients often complain about doctors not taking their complaints about “chemobrain” seriously.

“In the past, there was a lot of ignorance among doctors about chemo-induced cognitive problems. In some patients, problems can persist for up to two years,” Dr. Abraham said.

The researchers say that as many as 40 per cent of cancer patients undergoing chemotherapy complain of symptoms such as severe memory and attention deficits.

According to them, scientists previously suspected that the cancer might be the cause of this problem, instead of chemo drugs.

Dr. Abraham said that a study his team had unveiled earlier this year used MRI scans to document the extent of changes to the brain in women who received chemotherapy for breast cancer, and that the latest study further clarified the connection between drugs and memory loss.

Abraham, however, added: “At this point, we have no evidence to say that NAC is safe in patients who are getting chemotherapy. We need more studies to confirm the role of NAC in patients.”

The study has been published in the journal Metabolic Brain Disease.

The board is an independent body that makes classification decisions about films, computer games and publications.

According to Alexandra Greene, 31, a qualified lawyer on the Board since 2003, while graphic scenes of sex or violence may be confronting they are generally easy to classify.

She said that about 16 per cent of all the material that she and the other members of the board have to watch is usually “adult product” and then they have to classify it as G, PG, M, MA, R, X or refusing its classification.

“You get used to the adult product, you get used to seeing it and you know what to expect but I that makes you the opposite (of desensitised). You know what you’re supposed to look out for and when it comes up it jumps out at you,” News.com.au quoted, as saying.

She added: “It does get repetitive; it’s the same old thing. They go through the same scenes, vignettes and motions. They can only go so far with it, there is not much more they can invent.

“It’s one of the easier things to do because all we really have to look out for is violence and some fetishes that aren’t permitted.”

However, she said that the double entendres in children’s films such as Shrek and The Cat in the Hat’s title character calling a garden tool ” a dirty hoe” tread a line that can be hard to define.
